Submit Financial Reports
Information for Submitting Financial Reports
For financial statements ending January 1, 2006 and forward, use the following forms:
To upload financial statements through the department's web portal, using a user name and password:
New Health Plan Financial Reporting System
In partnership with the department's Office of Technology and Innovation, the Division of
Financial Oversight embarked on a major improvement to the Health Plan Financial Reporting
system. The major changes that affect health plans are:
- Users will now have unique logins. Your login will be a combination of the health plan name and
a new login and password that will be assigned to each individual.
- Reports no longer need to be submitted to DMHC all at once. Users will be able to start a report and
save their information if they need to return at a later date to complete the report.
- Users will be able to view a history of all the reports that have been submitted.
- Users will be able to view the status of a report and who the filing is assigned to at DMHC.
- New annual, quarterly and monthly reporting forms.
The new Health Plan Financial Reporting system is currently in the test phase.
We anticipate the new system going live on January 15, 2009. Click the links shown below

Training Sessions
Training sessions have been tentatively scheduled for December 8th and 10th in Sacramento. If you are interested in attending
please email HealthPlanReporting@dmhc.ca.gov by Friday, November 21st.
Test Website Information
A test website for the new system will be available until approximately Friday, December 12th, 2008. A link to the test website is
shown below:

Draft Reporting Forms
The proposed New Reporting Forms for your review, to be implemented in 2008.
Provide comments and feedback at HealthPlanReporting@dmhc.ca.gov regarding the New Reporting Forms.
New Login Information
Currently, a single login/password is used to submit the financial statements on the web
portal. The Plan's Financial Statement web portal single login/password will no longer be
active after December 31, 2008. An individual login/password is now required in order
to access the Health Plan Financial Statement Reporting application.
If you require access to the Health Plan Financial Statement Reporting application and:

You (or any other Plan representative(s)) currently have login/password access to the
DMHC web portal for one of the following listed applications, then send an email only to
Siniva Pedro (spedro@dmhc.ca.gov) requesting access
to the Health Plan Financial Statement Reporting application using your same assigned
individual access information. Current web portal applications: eFiling, Quarterly Grievance
Reports, Financial Solvency Reporting, and Claims Settlement Practices and Dispute Resolution

You currently DO NOT have login/password access to the DMHC web portal for any of the
above listed applications, please submit the
Electronic Filing Signature Verification Form,
checking the box that indicates HEALTH PLAN FINANCIAL STATEMENT REPORTING.
Please complete, print, sign (both individual and authorization signatures), and mail the original
hardcopy to the Department for processing. The individual login/password access
information will be mailed directly to the individual.
